Model Release:

In consideration for value received I hereby give the Photographer and parties designated by the Photographer, including clients, licensees, purchasers, agencies, and periodicals, the irrevocable right to use my name and/or a fictional name and photograph for sale to and  reproduction in any medium for purposes of, but not limited to: trade, advertising, display, exhibition, or editorial use.

I do (also) grant the Photographer the right to use my finished photos in a personal portfolio or as samples for display purposes only.

I will use the images I receive from this photo shoot for my personal use and/or self promotion only, and will not sell or assign the image(s). 

I have read this release, and understand and agree to its terms. I affirm that I am more than Eighteen (18) years of age, and have the right and ability to sign.

I hereby acknowledge receiving the following amount:                                              as payment in full for my talent fee and usage of my images.

I like to use a release that kind of covers all the bases, if it is just a TFP shoot and the model  does not want to give a blanket release I can cross out the first paragraph and we are ready to rumble.  If you intend to publish and there is any nudity involved you must have two forms of ID on file with the release.  Otherwise you might be liable for a huge federal fine.  So I just make it practice to always get copies of ID, then I don't forget the one time I really need it.  Also get the release signed before you start to shoot.  Once you pick up that camera signing a release is the last thing on your mind.

Depending on you and your needs adjust the wording.  This is a simple release that covers 90% of the situations.  Feel free to modify it to fit your needs.
